What is KIDS Transition Keyworker Service?

We offer advice, information and support for disabled young people aged 13 - 25 years and their families.

We provide a single point of contact for families and professionals to promote communication and understanding between all those concerned.

The Transition Keyworker can also provide intensive transition support for an average of six months for the young person as they move onto further education or leave school

We aim to deliver a high quality service to disabled young people living in the local area in partnership with their parents/carers


Who can use the service?

Disabled young people and those with complex health needs, their parents and carers


How can the Transition Keyworker Service Help?

We can help disabled young people to make a successful transition to adult services

Maximise opportunities for young people, for example in education, leisure and housing

We provide information regarding relevant provision and services available in the local area

Offer support and advocacy for young people and their parents/carers at transitional reviews

Work together with parents, connexions, schools and other agencies to ensure that the young persons needs continue to be met

Co-ordinate and arrange meetings between parents and professionals if required

Provide information on benefits, funding and grants

Offer advice and information allowing young disabled people and their families to make informed decisions

 

Opening Times: 09.00 to 17.00 Monday to Friday

Cost of Service: This service is free

How to Refer: There is an open referral system with consent from the young person or their family.  Age range 13 – 25.  Referral form available on request using the contact details below.
