Key Working

 

The key worker service is offered to those families who have a child with severe and complex needs and who have a number of professionals involved with their child. The key worker will work in partnership with families to oversee and co-ordinate the delivery of these services and will provide families with a single point of contact for information, advice, emotional and practical support.

A key worker will:

  • Be the first point of contact for advice and information.
  • Provide support to your family.
  • Will signpost you to relevant information/services/agencies.
  • Will be the first point of contact for other professionals/agencies working with you.
  • Will visit/contact you on a regular basis at a time & place agreed between you & the key worker.
  • Will identify gaps in services & ensure appropriate referrals to services
  • Will attend review meetings with you.
  • Will monitor current plans for your child.
  • Will ensure equality & promote you, the parents/carers, as equal partners.
  • Will be available during normal working hours.
